I hereby grant permission to Attitude Youth Charitable Trust to record and use audiovisual materials featuring myself for educational and promotional purposes. I understand and agree to the following terms and conditions: Consent: I consent to Attitude photographing, recording, filming, videotaping, and/or making audio recordings of me during my participation in Attitude's programs and activities. Ownership: I acknowledge that Attitude is the sole owner of all recorded materials. Attitude has the right to edit, duplicate, distribute, and use these materials for educational, promotional, and fundraising purposes. Release of Liability: I release Attitude, its officers, directors, employees, and representatives from any and all claims, damages, or liabilities arising out of the use of the recorded materials, including any misuse, distortion, or alteration of such materials. Compensation: I understand that I will not receive any financial compensation for the use of my image or voice in the recorded materials. Privacy Act 2020 (New Zealand): I understand that Attitude will collect, store, use, and disclose my personal information in accordance with the Privacy Act 2020 of New Zealand. Attitude will take reasonable measures to protect my privacy and will not disclose personal information that could identify me without obtaining separate written consent, except as required by law. Consent Revocation: I understand that I have the right to revoke this consent in writing at any time. However, revoking consent does not apply to any materials produced prior to the date of revocation. Governing Law: This Talent Release Form shall be governed by and construed in accordance with the laws of New Zealand.
